How referrals work
Send a patient. We'll take it from there.
We work the way busy referring providers need us to: clear handoffs, fast scheduling, and notes back to you within a week.
Send the referral
Fax the referral and any relevant chart notes to (918) 455-7546, or use your EMR's referral function with our practice info on file. Include patient name, DOB, insurance, and reason for referral.
We contact the patient
Our team reaches out within one business day to schedule. We accommodate same-week appointments for urgent primary care needs.
Closing the loop
Visit notes are sent back to the referring provider within 5 business days. Critical findings are communicated same-day. Records can be sent via fax, secure email, or HIE if your system is connected.
